WCN'24 The Journey of a Patient with Kidney and Cardiovascular Diseases: From Detection to Referral to Co-management

Case simulation of the management of a patient with kidney and heart diseases. We will use a clinical case (presented by moderators) starting with the diagnosis of kidney and heart problems and follow the journey of the patient from primary care to cardiology/nephrology co-management.
