This special episode of the Onconephrology Case Presentation Series is presented by three fellows, Drs. Api Chewcharat, Alvin Hu, and Si Yuan Khor from the Brigham and Women’s Hospital and Massachusetts General Hospital/Harvard Medical School and is moderated by Dr. Raad Chowdhry, Medical Director of Onconephrology at the Brigham and Women’s Hospital and Dana-Farber Cancer Institute. Together, they present a unique and layered case that exemplifies the complexity of kidney disease in patients with hematologic malignancies.

This session explores a 56-year-old woman whose presentation initially suggested a straightforward hypertensive crisis, but whose evaluation quickly revealed a deeper interplay of paraproteinemia, acid–base disturbances, and ultimately light chain cast nephropathy.
